Methods 73 patients of total upper lip skin defects have been treated in our department since September, 2002. The methods which we used include free skin grafting, local bilateral nasolabial flap, expanded forehead flap, expanded scalp skin flap, upper limb tubed skin flap and scapular skin flap extending to the upper arm.

自2002年9月起，我科共收治各种原因造成的全上唇皮肤缺损患者73例，分别应用皮片游离移植、鼻唇沟局部皮瓣转移、额部扩张皮瓣带蒂转移、头皮扩张皮瓣带蒂转移、上臂管形皮瓣和向上臂延伸的肩胛皮瓣转移修复。

Methods: Between 1984 and 1996, there were 44 cases of head and neck cancer that underwent radical surgery and had the pharyngealdefect reconstructed with a muscular flap. 2007年5月－2008年10月应用胫后动脉内跺上穿支筋膜蒂皮瓣修复足踝部软组织缺损13例，术前彩色超声多普勒行内深上穿支探测并体表标记，在解剖学基础上，皮瓣轴线在胫骨内侧裸后与内踝、跟腱间中点的连线，旋转点设计在穿支穿深筋膜处，深筋膜上游离皮瓣，术中可根据受区情况适当修整皮瓣厚度。

The patients were treated with reversal flow axial composite auricular island flap based on supraorbital artery or supratrochlear artery.results6 cases were treated with the technique, only one case underwent epidermal blisterand, and the partial skin flap necrosis, the necrosis area of flap was 1.0cm×2.0cm, 5 cases were successfully repaired with satisfactory.conclusionthe island flap could be an ideal way for the one stage reconstruction of large alar defects, which is recommended to be widely used.

结果：在临床上将该方法应用于6例病人，除1例出现表皮水疱，后皮瓣部分坏死，面积约1.0 cm×2.0，其余5例取得满意的临床效果。结论：反流轴型的耳廓复合组织瓣能ⅰ期修复大面积鼻翼缺损，是一项极具推广的修复手段。

Methods To tind the four kinds of methods that deal with the bare wound area on the skin flap pedicle,by reviewing operative methods that hand's skin imperfect is repaired by distant smaller random pattern skin flap（No including nearer fingers skin flap）in the treatment process of hand's injury in the author's hand surgery in the nearer three years,the four kinds of methods are:the pedicle is swed up to tube;Accepted wound skin edge nearly the pedicle and supplying wound skin edge are sewed up;Theskin flap's pedicle injury area is bared,and then bind up by Vaseline gauze;The fourth method is replanting free skin on the pedicle's injury area.

笔者复习了在手外科近3年来，在手外伤患者治疗过程中，应用远位（如胸腹、鱼际侧或另一肢体）微小随意带蒂皮瓣（是指皮瓣宽度在2.0cm以内的小皮瓣）修复手指部皮肤缺损的手术方法，发现在处理皮瓣蒂部处裸露创面上存在着4种不同的方法，这4种方法分别是：将蒂部缝合成管状，将受区蒂部侧皮缘与供区伤口缝合，将皮瓣蒂部创面裸露，用凡士林纱布包扎，第4种方法是在皮瓣蒂部创面上移值游离皮片。
