Abstract］ Objective To evaluate the role of three-dimensional （3D） digital subtraction angiography in the diagnosis and treatment of symptomatic intracranial arterial stenosis.Methods Stent-assisted angioplasty was performed on 30 patients with symptoms of intracranial artery stenosis by 3-dimesion（3D） method.These imagings were reviewed by different locations to get information of optimal projection angle in 2D angiography.

目的 探讨三维重建技术在颅内动脉狭窄的诊断和治疗中的应用方法对使用具有三维重建功能的美国Advantx LCN+型X线血管造影机进行的30例症状性颅内动脉狭窄的患者的支架成形术进行回顾性分析。

结果 术后随访5～77个月，平均32个月，按nakai标准评定疗效：优254例，良41例，可5例（其中在外地做过椎间盘镜术3例，木瓜酶溶核术后1例，而后到本院行此手术），差0例。结论影像学检查为腰椎管狭窄症，黄韧带肥厚的患者，体征与狭窄的定位，在腰椎间盘突出症和腰椎管狭窄症手术方式和适应证的选择上起重要作用。

结果：1、不同的扫描技术参数所获得的造影图像质量不同，注射速率3.0ml/s组以及125m1对比剂总量组比较适合国内患者。2、多层螺旋CT冠状动脉造影可显示冠状动脉的主要分支，尤其是冠状动脉的近、中段，显示率在90％以上。3、多层螺旋CT冠状动脉造影对狭窄病变的敏感性为87.3％，能检出冠状动脉造影不易发现的细小斑块，判断狭窄的原因及斑块的特征。4、多层螺旋CT冠状动脉造影可显示搭桥血管的形态、位置及通畅程度，显示桥血管及支架的内腔，安全便捷。5、多层螺旋CT显示冠状动脉畸形准确性高（3/3），多种成像方法可弥补冠状动脉造影显示的不足。6、仿真内窥镜可显示冠状动脉内膜和内腔，显示狭窄斑块的性质。

结果 在56例行MSCT冠状动脉造影的患者中1例由于患者有严重的窦性心率不齐，重建图像质量欠佳，其余患者均成功显示冠状动脉各主支（左主支、回旋支、前降支右支冠状动脉）及部分3、4级分支，且可显示所有冠状动脉支架的形态、位置，其中18例患者的31个支架术后出现再狭窄，所有行MSCT冠状动脉造影检查的患者无一例出现并发症，所有CT诊断支架术后再狭窄的患者同时行选择性血管造影检查，其中14例患者的25个支架有支架术后再狭窄，CT诊断的准确率为80.65%。
