Methods: a comprehensive analysis of 20 cases with acute left ventricular failure as the first manifestations of acute subendocardial myocardial infarction patients with a history of 20 cases and clinical characteristics, according to ECG, dynamic changes of enzymes, Killip classification of cardiac function and cardiac color report indicators of cardiac function, acute acute subendocardial myocardial infarction with acute left heart failure occurred in the characteristics and causes.

综合分析20例以急性左心衰竭为首发表现的急性心内膜下心肌梗死20例病人的病史和临床特点，根据心电图、心肌酶的动态变化、 Killip 心功能分级和心脏彩超报告的心功能指标，探讨急性急性心内膜下心肌梗死发生急性左心衰竭的特征和原因。

我院icu 2001年5月～2004年12月采用机械通气抢救aopp并发呼吸衰竭患者37例，除2例因服毒量大、中毒时间长引起多脏器功能衰竭死亡之外，余均脱机成功痊愈出院，现将护理体会总结如下。

Results: The causes of 56 patients who died were as follows: 29 patients died from the complication of respiratory system,11 patients died from anastomotic leaking,6 patients died from hemorrhagic shock,3 patients died from multiple organ failure, 3 patients died from whole body failure caused by chylopleura, 1 patient died from empyema and gastric necrosis, 1 patient died from sudden cardiac death, 1 patient died from heart failure, 1 patient died from strangulating intestinal obstruction and toxic shock caused by diaphragmatic hernia.

结果：56例死亡患者的死亡原因：肺部并发症29例（51.8％），吻合口瘘11例（19.6％），失血性休克6例（10.6％），多器官功能衰竭3例（5.4％），乳糜胸致全身衰竭3例（5.4％），胃后坏死、脓胸1例（1.8％），心脏性猝死1例（1.8％），心力衰竭1例（1.8％），膈疝引起绞窄性肠梗阻中毒性休克1例（1.8％）。

Objective To Study the scopolamine in the treatment of respiratory and circulatory failure caused by severe brain trauma or cerebral infarction. Methods The 31 respiratory and circulatory failure patients caused by severe brain trauma or cerebral infarction (treatment group,of which 18 cases of traumatic brain injury,cerebral infarction 13 cases) had both the conventional therapy and scopolamine treatment. With the same period 34 patients were treated only with the conventional therapy (control group,of which 19 cases of traumatic brain injury, cerebral infarction 15 cases).The two groups had no significant difference in gender, age and APACHEII score.

目的 研究东莨菪碱在重型脑外伤及脑梗塞致呼吸循环衰竭中的治疗效果；方法对31例重型脑外伤及脑梗塞致呼吸循环衰竭患者（治疗组，其中脑外伤18例，脑梗塞13例）在常规治疗的同时应用东莨菪碱治疗，并与同期收治的34例仅以常规治疗患者（对照组，其中脑外伤19例，脑梗塞15例）比较，两组在性别、年龄、APACHEII评分等差异无显著性。

结果显示60％患者在4～90天内撤机并拔管，存活；只有5.9％在1～10天内死于休克、急性肾功能衰竭、气压伤，而另34.1％需要长期MV支持30～2083天（229±136天），最后分别死于医院内感染、营养不良、多器官功能衰竭、MV故障等。
