Method］reversed sural neurofasciocutaneous flap with muscle was applied for repairing the soft tissue defect simultaneous open fracture of tibia lower section 6 cases,the soft tissue defect simultaneous chronic osteomyelitis of tibia lower section 5 case,the soft tissue defect of sole of foot simultaneous calcaneus epicarp 3 case, the soft tissue defect of sole of foot simultaneous calcaneus osteomyelitis or dead space 2 case. the scope of the flap was 18 c m×13 cm～11 c m×7 c m and that of the gastrocnemius flap was 4 cm×3 cm～9 c m×6 cm,the thickness was 1～3 cm,the skin flap was bigger than the muscle flap.

方法］自2004年6月以来，应用带肌肉的腓动脉穿支蒂腓肠神经营养血管皮瓣修复小腿下段及足踝部软组织缺损16例，其中胫骨下段开放性骨折伴软组织缺损6例，胫骨下段软组织缺损伴慢性骨髓炎5例，足后跟足底软组织缺损伴跟骨表层组织缺损3例，跟骨骨髓炎伴窦道2例，皮瓣面积30 cm×10 cm～6 cm× 4.5cm，切取的腓肠肌瓣面积4 cm×3 cm～9 cm×6 cm，肌肉厚度1～3 cm，皮瓣面积比肌瓣的面积要大。

Objective To investigate the application of anterolateral thigh fasciocutaneous flap on abdominal chronic ulcer,and provide evidence for its clinical use. Methods From Oct 2005 to Dec 2008,5patients with abdominal ulcer caused by different reasons were repaired with anterolateral thigh fasciocutaneous flap, the flap pedicled with rami descenden of lateral circumflex femoral artery was designed according to the size of abdominal defect,the vessel pedicle was dissociated adequately, the flap was transferred to the abdominal wound under appropriate tension through tunnel, the donor site was sutured directly or repaired with skin-grafting. Results The anterolateral thigh fasciocutaneous flap survived well in all 5 patients,there were no other complication such as skin necrosis.

目的：探讨股前外侧皮瓣在下腹部慢性溃疡中的作用，为其临床应用提供依据；方法：2005年10月～2008年12月间，对5例不同原因引起的下腹部慢性溃疡患者进行修复，术前根据腹部组织缺损的大小设计以旋股外侧动脉降支为血管蒂的岛状股前外侧皮瓣，充分游离血管蒂部，在合适张力下经过皮下或开放隧道将皮瓣带蒂转移至下腹部创面，供区直接缝合或植皮修复。

The patients were treated with reversal flow axial composite auricular island flap based on supraorbital artery or supratrochlear artery.results6 cases were treated with the technique, only one case underwent epidermal blisterand, and the partial skin flap necrosis, the necrosis area of flap was 1.0cm×2.0cm, 5 cases were successfully repaired with satisfactory.conclusionthe island flap could be an ideal way for the one stage reconstruction of large alar defects, which is recommended to be widely used.

结果：在临床上将该方法应用于6例病人，除1例出现表皮水疱，后皮瓣部分坏死，面积约1.0 cm×2.0，其余5例取得满意的临床效果。结论：反流轴型的耳廓复合组织瓣能ⅰ期修复大面积鼻翼缺损，是一项极具推广的修复手段。
