输卵管造口术

Hydrosalpinx is detrimental to the outcome of IVF-ET and may cause the increase of abortion,appropriate procedure should be adopted to deal with the problem before the IVF-ET and may improve the outcome of IVF-ET and may decrease the abortion rate.Compared with the salpingectomy,the proximal tubal ligation with distal tubal salingostomy may be a better option in this situation and it may cause less interference to the ovarian function.

输卵管积水对IVF-ET有不良影响，增加了流产率及异位妊娠率，在IVF-ET前对输卵管积水进行适当的处理有助于提高临床妊娠率，降低流产率；与输卵管切除术相比，输卵管近端结扎伞端造口术对卵巢功能影响较小，不增加流产率及异位妊娠率。

Objective To explore the value of laparoscopic operation in the treatment of gynecologic diseases.Methods Retrospective analysis of 170 cases of laparoscopic surgery in ynecology.53 cases in the salpingocyesis group,49 cases salpingotomy,4 cases salpingectomy;48 cases in the ovarian cyst group,35 cases enucleation and repair of ovary,8 cases ovary sutures atches,3 adnexnectomy and 2 perforation of ovary;52 cases in the uterine problems group,10 cases LM5 cases CISH,11 cases LSH（laparoscopic supracervical ysterectomy）,14 cases LTH（laparoscopic total hysterecotomy）,12 cases LAVH laparoscopically assisted vaginal hysterectomy）;17 cases in the infertility group were studied in terms of lysis of pelvic adhesions and salpingostomy.

目的 探讨妇科腹腔镜手术的临床应用价值方法回顾性分析170例妇科腹腔镜手术病例，其中异位妊娠组53例，行输卵管切开取胚49例，输卵管切除术4例；卵巢疾患组48例，囊肿剥除术及卵巢成形术35例，卵巢缝合修补8例，患侧附件切除3例，囊卵巢多点打孔2例；子宫疾病组52例，行肌瘤剥除10例，改良CISH术5例，子宫次全切除术11例，子宫全切术14例，腹腔镜辅助阴式子宫切除12例；不孕症组17例，行盆腔粘连分解输卵管伞造口术。

Methods Two hundred and eight cases of laparoscopic surgery in gynecopathy were analyzed retrospectively, including salpingotomy, salpingectomy, salpingostomy, enucleation of pelvic masses, CISH, remoral of hysteromyoma, lysis of pelvic adhesions.

回顾性分析208例妇科腹腔镜手术病例，主要包括输卵管切开胚胎取出术、输卵管切除术、输卵管造口术、盆腔肿物切除术、子宫切除术、子宫肌瘤剔除术和盆腔粘连松解术等15种手术。

The data were divided into four groups,A group included 57 cycles:a proximal tubal ligation with distal tubal salingostomy had been introduced to the patients before the IVF.B group included 55 cycles,a salpingectomy had been introduced to the patients before the IVF.C group included 52 cycles:no surgical procedure was adopted before IVF-ET.The control group was defined as D group and included 78 cycles of IVF-ET.

回顾性分析2007年3月至2009年3月在沈阳204医院生殖中心因输卵管性不孕接受IVF-ET 治疗共242周期的临床资料，根据IVF-ET前输卵管情况分4组，A组：57个周期，采用输卵管近端结扎伞端造口术；B组：55个周期，已行输卵管切除术；C组：52个周期，积水未行手术组；D组：78个周期，单纯输卵管阻塞，作为对照组。

Methods123 cases of laparoscopic surgery in gynecopathy were analyzed retrospectively, including salpingotomy, salpingectomy, salpingostomy, enucleation of pelvic masses, CISH, remoral of hysteromyoma, lysis of pelvic adhesions 15 kinds.

方法回顾性分析123例妇科腹腔镜手术病例，主要包括输卵管切开胚胎取出术、输卵管切除术、输卵管造口术、盆腔肿物切除术、子宫切除术、子宫肌瘤剔除术和盆腔粘连松解术等15种手术。

TV laparoscopy was applied in 64 gynecology operations, including salpingectomy, salpin-gostomy, dilatation of fimbriae tubae, tubal ligation, ovariectomy, adnexectomy, ovariocystectomy, oophoro-plasty, ovariopuncture, hysteromyomectomv and so on.

应用电视腹腔镜施行妇科手术64例，包括输卵管切除术、输卵管造口术、输卵管伞端扩张术、输卵管扎管术、卵巢或附件切除术、卵巢囊肿剥出成形术、卵巢穿刺术、卵巢冠囊肿切除术、子宫肌瘤挖除术及盆腔粘连分解术等。

Including salpingectomy,salpingotomy, salpingostomy, excision and enucleation of ovarian masses,repair of ovary, perforation of ovary, adnexnectomy, removal of hysteromyoma, supracervical hysterectomy, LAVH, CISH, lysis of pelvic adhesions, uterine suspension, uterosacral ligment transection, tubal sterilization, peritoneovaginoplasty.

手术包括输卵管切除术、输卵管切开吸胚术、输卵管造口术、卵巢肿块的剔除及切除术、卵巢修补术、卵巢多点打孔术、附件切除术、子宫肌瘤剔除术、子宫次全切除术、腹腔镜辅助的子宫切除术、腹腔镜筋膜内子宫切除术、盆腔粘连松解术、子宫悬吊术、骶韧带切断术、绝育术、取环术、腹膜阴道成形术等。

The lower pregnancy rate was found in Group C,the highest abortion rate and ectopic pregnancy rate were found in Group C was 21.15% and 7.69% repectively,and was statistially different from that in control group (P.05).Conclusion Hydrosalpinx is detrimental to the outcome of IVF-ET and may cause the increase of abortion,appropriate procedure should be adopted to deal with the problem before the IVF-ET and may improve the outcome of IVF-ET and may decrease the abortion rate.

输卵管积水对IVF-ET有不良影响，增加了流产率及异位妊娠率，在IVF-ET前对输卵管积水进行适当的处理有助于提高临床妊娠率，降低流产率；与输卵管切除术相比，输卵管近端结扎伞端造口术对卵巢功能影响较小，不增加流产率及异位妊娠率。

Methods The data of 242 cycles of IVF-ET which were accomplished in the Reproductive Medicine Center of 204 hospital during March 2007 and March 2009 with a history of hydrosalpinx were analyzed retrospectively.The data were divided into four groups,A group included 57 cycles:a proximal tubal ligation with distal tubal salingostomy had been introduced to the patients before the IVF.
