Methods Fifty eight cases suffered avulsion injury were treated by debridement and orthotopic transplantation of avulsed flap within 6 hours, 31 of them were received intravascular low level He-Ne laser irradiation and routine treatment, and 27 of them were received routine treatment as control group.

选择伤后6小时内行清创、撕脱瓣原位回植术后撕脱伤患者58例，其中治疗组31例应用低能量He-Ne激光血管内照射加常规治疗，对照组27例仅用常规治疗。

Since the graft complex effectively combines the advantages of both full-thickness flaps and ultrathin flaps and maintains the maximum morphology of the avulsed skin as well, it can improve the survival of avulsed skin. Moreover, the operation is easy and safe.

本术式有效地整合超薄皮瓣和全厚皮片移植的优点，最大限度地保存了撕脱皮肤的形态学特点，提高撕脱皮肤成活率，手术简单安全易行。

Results The survival area and quality of avulsed flap in the experimental group were superior to that of control group after 15 days of operation, and the hemorheological items were markedly changed at 5 days after operation. Conclusion The better flap survival after orthotopic transplantation in avulsion injury can be improved by intravascular low level He-Ne laser irradiation through changed superoxide dismutase activity and hemorheological items in optimal irradiation intensity.

结果 术后15天治疗组撕脱瓣的成活面积、成活质量均明显优于对照组P结论低能量He-Ne激光血管内照射的生物学效应存在最佳的照射强度总量，在最佳的照射强度条件下，通过改变细胞内超氧化物歧化酶活性及血液流变学参数促进原位回植术后的撕脱瓣良好成活。

The graft complex was transplanted onto the wound surface. Results In all the patients, 90% to 100% of the avulsed skin survived, with an average of 94.1%. There was no infection. In the 12 cases that had been followed up for 2 months to 3 years, the grafted skin showed normal shape, color, texture, sensation, and function.

结果 本组21例，撕脱皮肤占体表面积2%~6%，平均3.3%，成活皮肤面积占撕脱皮肤90%~100%，平均94.1%，无创面感染，随访12例，一年后回植皮肤质地柔软，无明显挛缩，功能基本恢复。

Methods The animal model of skin avulsion injury was replicated, and the expression of CD18mRNA in avulsed skin tissues was detected by RT-PCR method; and the amount of neutrophilic granulocyte in tissues was detected by MPO method in order to analyze the tendency of neutrophil of avulsed tissues after trauma.

复制猪皮肤撕脱伤模型，采用RT-PCR方法检测伤后不同时间撕脱组织中CD18mRNA的表达；用髓过氧化酶（Myel operoxi daBe enzyme，MPO）法测定组织中中性粒细胞数量，研究伤后撕脱组织中中性粒细胞的趋化情况。

METHODS: Form October 1992 to July 2001, we treated 7 cases of avulsed scalp, which had been wounded more than 12 hours and accompanied with shock and head wound, with split thickness scalp skin grafting. RESULTS: Except for partial necrosis of scalp in center of bare area of skull, more than 90% of grafting split thickness scalp skin survived in 4 cases and more than 80% in 3 cases, and presented satisfactory appearance during following up.

1992年 10月～ 2 0 0 1年 7月对 7例头皮撕脱在 12小时以上，撕脱面积达 2 %体表面积，头部创面和头皮伴不同程度污染，同时分别伴有休克和颅脑外伤者，通过将撕脱的头皮制成条状中厚皮片游离回植，覆盖头部创面。
