Six patients had the history of delayed defecation. two cases were misdiagnosed as sigmoid torsion and 6 cases were admitted as acute intestinal obstruction. colostomy was performed first in 7 cases. surgical procedures included swenson operation in 3, duhamel in 2, rehbein in 3, soave operation in one and stapler anastamosis in 2 cases. presacral infection and perianal fistula occurred in one case with swenson operation.

结果 男5例，女7例，年龄14～56岁，6例有胎粪排出延迟病史，2例误诊为乙状结肠扭转，6例诊断为急性肠梗阻，其中7例行结肠造瘘，3例行swenson手术，2例行duhamel手术；3例行rehbein手术；1例行soave手术；外院术后复发2例，swenson手术1例术后出现骶前感染肛周瘘管，余术后排便功能良好。

［Objective］To explore the technique of arthroscopy plus radiotherapy in the treatment of pigmented villonodular synovitis of the knee and to evaluate its clinical value［Method］A total of 14 cases of pigmented villonodular synovitis of the knee were treated from January 2000 to July 2003Intraarticular excision under arthroscope with a combined application of routine intrusments and Bipolar radiofrequency was performed on all these 14 cases,and postoperative radiotherapy for diffused pigmented villonodular synovitis of the knee,13 patients were followed up for 10~45 months,and were evaluated for range of motion,pain,swelling effusion［Result］Thirteen cases according to the International Knee Documentation Committeecriteria,subjective knee score was (587+64) and (865 ±57) respectively before surgery and at the last followup［Conclusion］In case of pigmented villonodular synovitis of the knee joint,arthroscopical synovectomy can make good surgical results,and postoperative radiotherapy is an effective treatment for diffused pigmented vilionoclular synovitis of the knee

作者：王晓旭，廖瑛，黄华扬，席雅文，李朝旭，韩志关节镜；放射治疗；膝关节；滑膜炎；色素绒毛结节性摘要：［目的］探讨采用关节镜结合放射治疗膝关节色素沉着绒毛结节性滑膜炎的方法及其临床应用价值。［方法］2000年1月～2003年7月，共收治14例膝关节色素沉着绒毛结节性滑膜炎患者，采用关节镜下常规器械加双极射频进行关节内病变切除，术后弥漫性色素沉着绒毛结节性滑膜炎辅以放疗，13例通过10~45个月的随访，了解患膝活动度、疼痛、肿胀情况。［结果］13例随访患者，术前国际膝关节评分委员会膝关节功能主观评分为（587±64）分，最后随访时，IKDC膝关节功能主观评分为（865±57）分。［结论］通过关节镜能完成膝关节色素沉着绒毛结节性滑膜炎滑膜较彻底切除，对弥漫性膝关节色素沉着绒毛结节性滑膜炎辅以术后放疗是一种有效方法。

Abstract］ objective to explore the experience in the diagnosis and treatment of small bowel diverticular surgical complication.methods 24 cases of small bowel diverticular were retrospectively analysed.results the diagnosis was made preoperatively in 4 duodenal diverticula and only 1 jejunal diverticula.clinical complications included obstruction in 11 cases,bleeding in 4 cases,perforation in 4 cases.23 cases were cured.1 case was died of mof.all the diverticular with complications had histopathological signs of inflammation and 3 had signs of ulcer and 4 had the resected.conclusion the small bowel diverticular is rare.it may diagnose except common reason underwent gastrointestinal bleeding or obstruction.the gi is an effective way of examinations.operation is effective for small bowel diverticular disease.

回顾性分析24例小肠憩室病外科并发症的临床资料。结果术前6例十二指肠憩室中4例确诊，18例空回肠憩室仅1例确诊。小肠憩室伴小肠梗阻11例，憩室出血4例，憩室穿孔4例。23例治愈，1例死于多脏器功能衰竭。有并发症的憩室病理都有炎症表现，5例有溃疡，4例有异位黏膜组织。结论小肠憩室病比较少见。在胃肠出血或梗阻的情况下，可采取对常见原因排除的方法进行诊断。在不需要急诊手术的情况下，钡餐造影是最有效的检查。手术是可靠有效的治疗方法。

Cases underwent bilateral TMJ reconstruction, bone graft and genioplasty in order to advance the jaw. Bimaxillary surgical procedure was completed in 3 cases, and distraction osteogensis and TMJ reconstruction in 6 cases. Al patients gained good appearance, 29 patients can open their mouth larger than 3 cm, and one case had recurrent TMJ ankylosis. 29 cases'AI＜5 and oxygen saturation degree ＞90%(included the patient that had recurrent TMJ ankylosis).

结果 本研究有15例患者接受患侧下颌升支倒置TMJ重建、植骨前移，健侧升支矢状劈开、下颌前移和颏前移成形术；其中有3例行二期的上颌的Le Fort Ⅰ截骨术。6例行双侧TMJ重建、植骨前移下颌和颏成形术。3例施行同期双颌截骨前移和颏成形术；6例行单或双侧TMJ重建、牵引成骨术。

All fractures were classified according to Hardegger system:The scapular body fracture in 9 cases,the surgical neck fracture in 3 cases,the anatomical neck fractere in 1 case,the glenoidal lip fracture in 3 cases,the scapular spine fracture in 2 cases.

The imaging manifestation of BUS and IVP were restrospectively analysed in accordance with surgical findings.Results For 12 CRDA cases with severe hydronephrosis,BUS was diagnostic in 9 cases and suggestive in 2 cases,while IVP was only suggestivc in 4 cases;For 11 cases with mild to moderate hydronephrosis,BUS made correct diagnosis for 10 cases,IVP was correct in 5 cases;For 8 CRDA of maldeveloped type,BUS made diagnosis for 5 and was suggestive for 1,IVP was correct for 2;For 3 cases CRDA of developed type,IVP was correct for all while BUS was correct only for 1 case.
