Objective: To provide anatomic data of vessels at the anterior aspect of lumber vertebrae for transperitoneal approach lumbar anterior minimally invasive surgery. Methods: This study investigated 30 human cadavers and 120 patients' magnetic resonance images, to study the major vascular anatomy of the anterior aspect of lumber vertebrae, and observe the relationship of the confluence of the common iliac veins with vertebral body or disc, as well the relationship of left iliac vein and L5~S1 disc.

目的：为经腹膜腰椎前路微创手术提供腰椎前方血管的解剖学依据；方法：选择30例防腐固定尸体标本及120例腰椎MRI断层数据，观察下腰椎区域髂总静脉的解剖学特点；根据腰椎前方血管的断层图片判断髂总静脉汇合点对应椎体的位置；L5~S1前方左髂总静脉对应椎间盘的位置。

Methods Fifty healthy SD rats were fed for twentytwo months in order to build the model of aging rats, the histology of femoral bone, tibial bone and lumbal vertebra was used to observe, then we evaluated the indexes which reflect the degeneration of intervertebral disc including the quantity of collagen type Ⅱ,Ⅹ of intervertebral disc by the immunohistochemistry methods, the relative area of vascular bud, the thickness of calcified layer and uncalcified layer on cartilage endplate and the indexes which reflect the osteoporosis including the thickness ratio of cortical bone, the relative area of bone trabecula, the bone density, the max stress and straining of bone.

50只健康纯种SD大鼠，喂养22个月，建立老龄大鼠模型，进行股骨、胫骨、腰椎等组织学观察，并评价椎间盘退变的相关指标(椎间盘软骨Ⅱ型和Ⅹ型胶原表达、软骨终板血管芽相对面积、软骨终板钙化层/非钙化层厚度)与骨质疏松的相关指标(骨皮质厚度指数、骨小梁相对面积、骨密度、骨最大应力和应变)的相关性。

Objective To explore the indication of treatment of lumbar disc herniation with collagenase Methods The surgical operations were made in 30 cases of the lumbar disc herniation treated with collagenase, all of them were uneffective.

目的进一步探讨胶原酶治疗腰椎间盘突出症的适应证。方法在2年中对30例用胶原酶治疗腰椎间盘突出症的患者因治疗无效，症状未改善，予以手术摘除术。

Results. No changes were observed at the segments located below the fusion. All the unfused segments above the fusion showed the same significant loss of disc height. Loss of disc height did not depend on fusion parameters, correlated weakly with age and length of follow-up, and correlated highly across adjacent unfused segments.

结果：在融合节段下方的未融合节段未发现任何改变，所有融合节段上方的未融合腰椎的椎间盘高度明显丢失，椎间盘高度的丢失与融合所使用的器械无关，与患者的年齡的随访的时间有一定联系，与距离融合节段的远近关系密切。

结果 术后随访5～77个月，平均32个月，按nakai标准评定疗效：优254例，良41例，可5例（其中在外地做过椎间盘镜术3例，木瓜酶溶核术后1例，而后到本院行此手术），差0例。结论影像学检查为腰椎管狭窄症，黄韧带肥厚的患者，体征与狭窄的定位，在腰椎间盘突出症和腰椎管狭窄症手术方式和适应证的选择上起重要作用。
