This case study shows that the interplay of patrilineal kinship and capitalism contributed to the transformations of divisions of domestic labor in post-war Taiwanese families. This article is about adoption of Watson's caring theory for a 38-year-old primigravida, who presented with early uterine contraction and was hospitalized for tocolysis. Clinical data was collected and assessed by interview, observation and body assessment during October 17th and November 15th, 2006. We found the client worried about the fetal health owing to premature labor and felt frustrated about limitations and demands of learning and accomadating herself to high-risk labor, resulting in anxiety, constipation and health-seeking behaviors.

本文在分享运用Watson关怀理论於一位高龄初孕妇因子宫早期收缩住院安胎之护理经验，笔者於民国95年10月17日至11月15日护理期间以会谈、观察及身体评估收集资料，发现个案担忧早发性分娩对胎儿健康造成威胁，及为适应及学习高危险妊娠的限制及需要，引发个案有焦虑、便秘、健康寻求行为的问题。

An interview guide, entitled "Maternal Perception of the Onset of Lactation", was also used to obtain milk secretion data. Results found a mean onset of lactation of 78.2±18.7 hours (range=42 to 121 hours). The prevalence of delayed lactation onset was 43.6%(n=58). Slighly over half of participants (56.9%, n=33) were primiparous. Parity, total second stage of labor duration, duration of second labor stage beyond one hour, and perinea wound pain were the most significant predictors of delayed lactation onset.

研究结果发现：(1)首次泌乳时间的平均值为78.2±18.7小时（范围42至121小时）；(2)产妇自觉延迟泌乳的发生机率为43.6%(n=58)，其中初产妇占56.9%(n=33)，经产妇占43.1%(n=25)；(3)产次、第二产程时间、第二产程时间大於一小时、第一到第三天会阴伤口疼痛分数，是延迟首次泌乳时间的重要预测因子。

This article studied differences in postpartum fatigue and birth outcomes between women who pushed immediately and those who delayed pushing during the second stage of labor. Data were collected from primiparous women in their 38th to 42nd gestational week who did not receive epidural analgesia during labor and were free of complications during pregnancy. Using a quasi-experimental design, 72 participants selected by convenient sampling were assigned based on individual participant's preference to either an experimental or control group. For the experimental group, pushing was delayed until the point after full cervical dilation at which the mother felt a strong physical pushing reflex, the fetal head had both descended to at least the +1 level in the pelvis and turned to the occiput anterior position, and uterine contractions were at least 30 mmHg. For the control group, the physician instructed mothers to begin pushing after full cervical dilation at the point when the fetal head was in the occiput anterior position and uterine contractions were at least 30 mmHg.

本研究目的在探讨初产妇女第二产程立即用力与延迟用力对产后疲惫及生产结果的影响，收案对象为怀孕38-42周初产妇女、待产期间未施打硬脊膜外麻醉，且怀孕期间无任何合并症，於待产期间向其解释研究目的及收案方法后予以收案，采类实验设计法，以方便取样并依个案之意愿分派於实验组及控制组，实验组於子宫颈完全扩张后等到强烈想用力的感觉、胎头位到达坐骨棘连线下1公分以下、胎头位置转到枕前位、子宫收缩规律强度至少30mmHg才教导向下用力；控制组於子宫颈完全扩张后即教导向下用力，共收案72位。
