ABSTRACT:BACKGROUNDTo study the methods and curative effects for Ⅲ degree rotator cuff injury treated by the openloop acromioplasty and rotator cuff neoplasty.

［背景］探讨应用开放式肩峰成形术及肩袖修补术治疗Ⅲ度肩袖损伤的方法及其临床疗效。

Objective To understand rotator cuff tear in aged patients, and the correct treatment of rotator cuff tear.

目的 增强对老年人肩袖撕裂的认识，以正确处理老年人肩袖损伤。

CASE REPORTSFrom July 2005 to January 2008, 16 cases of Ⅲ degree rotator cuff injury were treated by the openloop acromioplasty, and at the same time, after full loose of the injured rotator cuff, it was sutured on the greater tuberosity of humerus.

病例报告］自2005年7月—2008年1月间给16例肩袖损伤患者施行开放式肩峰成形术，同时将损伤的肩袖充分松解后，缝合在肱骨大结节上。

For detecting rotator cuff tear , the sensitivity , specificity , and accuracy of MR arthrography were 100 %, 100 %, 100 % respectively. For detecting complete rotator cuff tear , the sensitivity , specificity , and accuracy of MR arthrography were 100 %, 94 %, 97 % respectively.

肩关节MR 造影判定有无肩袖撕裂的敏感性、特异性和准确性都为100 %；诊断全层撕裂的敏感性、特异性和准确性分别为100 %、94 %和97 %。

Results: There were thirty-six small rotator cuff tears, forty-seen large isolated supraspinatus or combined supraspinatus and infraspinatus tendon tears, and twenty-two massie rotator cuff tears.

结果：其中36例有肩袖小的撕裂，47例有大的单独的冈上肌或者混合冈上和冈下肌腱撕裂，22例有肩袖完全断裂。

Results: MRI findings included: 13 cases of abnormality of the rotator cuff, 4 of abnormality of the glenoid labrum, 2 of abnormality of both the rotator cuff and long head of biceps tendon, 2 of abnormality of long head of biceps tendon,5 of subacromial bursitis, 4 of abnormality of bony structure.

结果：21例肩关节MRI扫描发现：肩袖病变13例、盂唇病变4例、肩袖病变合并肱二头肌长头肌腱病变2例、肱二头肌长头肌腱病变1例、肩峰下三角肌下滑囊脂肪消失和滑囊炎或滑囊积液5例、肩胛骨肿瘤1例。21例中同时伴不同程度肩关节退行性变及骨创伤4例。5例经开放性手术或关节镜证实，其余16例行保守治疗后临床随访。
